A single image is embedded losslessly
When you convert one image with no layout options set, the original bytes are copied into the PDF page rather than re-compressed. A print-quality scan stays print quality, and a photo is not put through a second round of compression.
As soon as you ask for something that changes the picture — several images, a specific page size, margins, scan cleanup, OCR, or a resize — the full layout engine takes over and renders the pages instead. That is the right behaviour, but it means the output is no longer a byte-for-byte copy of your input.
Two things that surprise people
Transparency does not survive. PDF pages have no alpha channel, so a transparent PNG takes on the page background. If you are converting a logo or a cutout, expect it to sit on white.
The result is a picture of your content, not text. Nothing inside it is searchable or selectable unless you turn on the OCR option, which adds a real text layer behind the image. For photographed documents, the scan-cleanup option straightens and brightens the page and makes a far more readable result.
